Premature Ejaculation Treatment
Overview
Premature ejaculation is a common sexual health condition in which ejaculation occurs sooner than desired, is difficult to delay, and causes distress, frustration, or problems with intimacy. It may be present from a person’s first sexual experiences or develop later after a period of normal ejaculatory control.
Premature ejaculation is not always caused by penile hypersensitivity. Physical, psychological, relationship-related, and medication-related factors may contribute, and some patients have more than one underlying cause. Effective management therefore begins with a confidential medical assessment rather than a one-size-fits-all procedure.

What Is Premature Ejaculation?
Premature ejaculation generally involves three features:
-
Ejaculation that consistently occurs sooner than desired
-
Difficulty delaying ejaculation during sexual activity
-
Personal distress, frustration, reduced confidence, or avoidance of intimacy
Guideline definitions distinguish between lifelong premature ejaculation, which has been present since a patient’s earliest sexual experiences, and acquired premature ejaculation, which develops after a period of satisfactory control.
Possible Causes
Premature ejaculation may be associated with:
-
Increased penile sensitivity
-
Anxiety or performance pressure
-
Stress or relationship difficulties
-
Erectile dysfunction
-
Prostate or pelvic health conditions
-
Hormonal or neurological factors
-
Changes in sexual routine
-
Certain medications or substance use
-
Previous negative or distressing sexual experiences
Because the cause varies, treatment should be based on a careful medical and sexual history.
Who May Benefit From Treatment?
A consultation may be appropriate for men who:
-
Regularly ejaculate sooner than they or their partner would prefer
-
Feel unable to control or delay ejaculation
-
Experience frustration, embarrassment, or reduced sexual confidence
-
Avoid intimacy because of the problem
-
Have symptoms that developed recently or have been present for many years
-
Have tried behavioral techniques without satisfactory improvement
Patients with pain, urinary symptoms, erectile dysfunction, genital skin changes, or a sudden change in ejaculation should undergo a medical evaluation to identify possible underlying conditions.
Hyaluronic Acid Injection of the Glans

In selected patients, hyaluronic acid may be injected beneath the surface of the glans penis with the aim of creating a cushioning layer that reduces tactile sensitivity.
The procedure is intended to decrease excessive sensory stimulation rather than interfere with the nerves themselves. It may also temporarily increase the volume of the glans.
This treatment should be considered carefully. Glans hyaluronic acid injection is not identified as a standard first-line therapy in major premature-ejaculation guidelines, and published evidence remains more limited than for behavioral therapy, topical anesthetics, counseling, and prescription medication.
Dr. Danino will discuss whether this option is medically appropriate, what evidence is available, and what alternative treatments should be considered first.
Recovery
Temporary swelling, tenderness, bruising, or discoloration may occur after treatment.
Patients are generally advised to:
-
Keep the treated area clean and dry
-
Avoid pressure or manipulation unless specifically instructed
-
Refrain from intercourse and masturbation for approximately one week
-
Avoid strenuous physical activity for several days
-
Contact the clinic if pain, swelling, discoloration, or urinary symptoms worsen
Early fullness may partly reflect swelling. The treated area generally settles over the following days to weeks.
Results
The response to premature-ejaculation treatment varies considerably between patients.
With hyaluronic acid injection, some patients may experience reduced sensitivity and improved perceived control. The degree and duration of improvement cannot be guaranteed.
Hyaluronic acid is gradually broken down by the body. Maintenance treatment may eventually be required, although the appropriate interval depends on the product used, the amount injected, individual metabolism, and clinical response.
Treatment should not be described as a permanent cure, and patients may still benefit from behavioral therapy, medication, counseling, or treatment of associated erectile dysfunction.
Risks and Possible Complications
Possible risks of glans hyaluronic acid injection include:
-
Swelling
-
Bruising
-
Tenderness
-
Temporary discoloration
-
Uneven distribution
-
Palpable lumps or nodules
-
Asymmetry
-
Migration of the filler
-
Infection
-
Prolonged numbness or altered sensation
-
Unsatisfactory improvement
-
Need for correction or dissolution of the filler
-
Rare vascular or tissue complications
Urgent medical assessment is required for severe or increasing pain, marked color change, blistering, fever, drainage, difficulty urinating, or other unexpected symptoms.
