
Dermal Fillers
Overview
Dermal fillers are injectable treatments used to restore facial volume, soften certain folds and wrinkles, enhance facial contours, and improve balance between facial features.
Many commonly used fillers are made from hyaluronic acid (HA), a substance naturally present in the skin and connective tissues. Hyaluronic acid attracts and retains water and contributes to tissue hydration and volume. Injectable HA gels are specifically formulated to provide support and volume beneath the skin.
Treatment can range from subtle correction of an individual area—such as the lips or chin—to a more comprehensive approach addressing age-related volume loss across several areas of the face.

What Can Dermal Fillers Treat?
Facial aging involves more than the development of wrinkles. Over time, changes occur in the skin, facial fat compartments, muscles, and underlying bone, altering facial proportions and contours.
Dermal fillers can be used strategically to address these changes or to enhance existing facial features.

The Dermal Filler Procedure
Treatment is performed in the clinic and is customized according to facial anatomy and the areas being treated.

Facial Assessment: The face is evaluated from multiple angles to assess proportions, symmetry, volume distribution, and areas of age-related change. An individualized injection plan is then developed.

Preparation: The skin is cleansed and the planned treatment areas are identified. Topical anesthetic may be used when appropriate, and many modern hyaluronic acid fillers also contain lidocaine to improve comfort.

Injection: Small amounts of filler are carefully placed at specific anatomical depths using a fine needle or cannula. The location, depth, and quantity depend on the treatment area and desired correction.

Assessment: Facial contour and symmetry are reassessed during treatment. Additional small amounts may be placed where appropriate rather than attempting excessive correction in a single session.
Most treatments can be completed within approximately 30 to 60 minutes, depending on the number of areas treated.

Hyaluronic Acid Fillers
Hyaluronic acid is one of the most widely used materials for temporary facial fillers.
Different HA fillers have different physical characteristics. Some are softer and more flexible, while others provide greater structural support. The product should therefore be selected according to where it will be injected and what it is intended to accomplish.
Products from established filler families such as Juvéderm® and Restylane® include formulations designed for different facial areas and injection depths.
An additional advantage of HA fillers is that they can generally be dissolved with hyaluronidase when medically necessary or when an appropriate aesthetic correction is required.
Results & Longevity
Improvement is usually visible immediately after injection, although swelling can temporarily exaggerate the result.
Most swelling and minor bruising improve over the following days, while the filler gradually integrates with the surrounding tissues.
Depending on the formulation and treatment area, hyaluronic acid filler results commonly persist for approximately 6 to 18 months, although some products and areas may last longer or shorter.
Areas with considerable movement, such as the lips, may metabolize filler more quickly than certain structural treatment areas.
Maintenance treatment can be performed when appropriate, but injections do not need to be repeated on a rigid schedule.
Recovery
Dermal filler treatment generally requires little downtime. Most patients can return to routine activities shortly afterward.
Temporary effects may include swelling, redness, tenderness, firmness, bruising, or mild asymmetry around the injection sites. These usually improve as the tissues settle.
Patients are typically advised to avoid manipulating the treated areas unless specifically instructed and to follow individualized aftercare recommendations.
Combining Dermal Fillers With Other Treatments
Dermal fillers primarily address volume and facial structure, so they may be combined with other treatments when several aspects of facial aging are present.
For example, botulinum toxin may address dynamic expression lines, while skin treatments or PRP may target different aspects of skin quality. Surgery may be more appropriate when significant tissue laxity cannot be adequately corrected with injections.
The goal is to select treatments according to the underlying anatomical problem rather than attempting to correct every sign of aging with filler.
